Fee: $349 annually plus $249 per additional member.

How it Works: This is a dental plan that allows individuals and families the ability to receive quality dental services at a discounted rate from the office’s standard fees. This is NOT an insurance plan, no claims are filed, and no payments are made to any other healthcare providers. Shipp Family Dentistry is not a licensed insurer, health maintenance organization, or other underwriter of health services. This plan may not be combined with any other offers, discounts, insurance, advertisements or CareCredit payments.
Membership: Once the annual fee is paid, you will be enrolled in the dental benefit program for 12 months. Members, spouses and dependent children under the age of 19 or full-time students up to age 23 are eligible. Children may be members only as dependents of adult members. You have the right to cancel at any time by submitting your written request to the front office administrator. The refund will be based on a pro-rated reimbursement of the annual fee paid.
